Our team provides comprehensive home health care including skilled nursing, rehabilitation and assistive care services wherever the patient calls home, whether that’s a traditional home setting or a residential care community such as an assisted (ALF) or independent living facility (ILF). We take a holistic approach to care that seeks to meet the specific needs of each home health patient.

We work closely with each patient’s physician to create a customized plan of care that focuses on disease management and patient education to improve quality of life and lessen the likelihood of future hospitalizations.

Our home health services

Skilled Nursing

Our highly skilled nurses have extensive knowledge in treating and educating patients in the home setting. We offer a wide range of skilled nursing services in areas such as disease-specific management and education for conditions like heart failure, COPD, diabetes and orthopedic rehabilitation. We also assist in medication management and administration, and provide dietary and nutrition management.

Physical Therapy

Our physical therapists help patients improve and restore their physical function and mobility levels at home. Physical therapy can help patients who have difficulty walking, trouble balancing, require the use of adaptive equipment or need rehabilitation post-operatively.

Occupational Therapy

Occupational therapy focuses on improving patients’ ability to perform activities of daily living. With the help of our occupational therapists, home health patients are able to maximize their fine motor skills and regain physical strength to help them accomplish daily tasks.

Medical Social Worker

Our social workers assess the social and emotional factors affecting each patient and their loved ones, and then assist in coordinating available community resources and short-term counseling.

Speech Therapy

Speech therapy focuses on helping patients maximize their skills and abilities to live independently. Our speech language pathologists are experts in oral and motor cognition and help patients improve their swallowing, mental processing and language skills.

Home Health Aides

Our home health aides are trained to assist patients with activities of daily living such as eating, bathing, grooming and taking vital signs. Under the supervision of our nurses and therapists, home health aides also support certain medical care activities like taking a patient’s pulse or blood pressure.
